Obtaining a referral
Referral to MIND is usually initiated by a referring physician who will commonly be a family medicine physician, but may practice any specialty including internal medicine, oncology, orthopaedic surgery, chiropractic or others. Feel free to communicate with your practitioner in regard to potential referral to MIND. You may also use the contact us form located on this Web site to request further information or to determine if you are an appropriate candidate for MIND referral. We will assist you as to how to proceed.
Once a clinician has decided to refer you to the Institute of Neuroscience, he or she will communicate with the Institute program assistant to initiate the process. The program assistant will subsequently communicate with you, the referred patient, to confirm that your referral has been received and to gather additional information. The program assistant will also provide direction as to how the referral will proceed. |
