Prior to your first visit
As soon as your referral is received, as mentioned, the Institute program assistant will contact you to help you understand the process that will follow. This may lead to direct referral to appropriate disciplines within the Institute, or on the other hand, it may be necessary to have MIND's clinical coordinator communicate directly with you to gain further clinical information. This may include additional medical history, including history of your referral problem, as well as very specific information about your current neurological signs and symptoms.
In some cases, this information will then be presented to an interdisciplinary team of practitioners, e.g. Spine Focus Group, for collaborative consideration and input in regard to your specific case. Based on this case study, the focus group will recommend a customized, initial plan of care for you. Following the focus group meeting, you will again be contacted and updated on the collaborative discussion and treatment plan. Appointments will be scheduled for referrals to the appropriate disciplines per your specialized treatment plan. You will be made aware of these appointments and the purpose of these referrals. |
