Preparing for your first visit

Please check your appointment schedule carefully for specific appointment times. It is highly recommended to arrive at least 15 minutes prior to your appointment, as it may be necessary to gather and verify information. For certain disciplines, it may be necessary for patients to undergo radiographic imaging prior to their visit. This may require even earlier arrival. Make sure you bring all insurance information with you to your first visit.

In some cases, you may be requested to complete additional medical forms or questionnaires prior to your arrival and it will be important that you bring these completed forms along to facilitate your visit. Additionally, if it has been requested, make sure you bring any medical records from other institutions and/or imaging studies, e.g., MRI or CT scans, x-rays, etc., with you to your first visit.

If you are coming to MIND for an outpatient procedure or treatment, the clinic or practitioner will give you instructions about arrival time, where to go, how to register and any necessary special preparation. Depending on what procedure or test is scheduled, you may be asked to have a family member or friend drive you home.
