Your health care responsibilities as a patient

So that we may offer you the best possible care, you should give our staff complete, up-to-date information concerning your health. It is very important that you, as a patient, become an active participant in your health care. Your active participation helps us to always ensure safe care. Important considerations and practices in this regard may include:

  • Learn as much as you can about your care and be certain that you understand every aspect of your care.
  • Communicate any questions or concerns you may have after your visit to the Mercy Institute of Neuroscience office or to your individual practicitioner.
  • Voice your agreement/disagreement with the care plan in a respectful fashion.
  • Be an active participant in every decision.
  • Provide complete information about past illnesses, hospitalizations, medications and other matters relating to your health, to the best of your ability.
  • Ask for written information about your care and/or medications that will better enable you to understand your condition or treatment.
  • Follow up on results of scheduled tests or procedures.
  • Bring along a family member or friend to help you understand information discussed during your visit.
  • Make sure you understand the different disciplines involved in your care, as well as the goals associated with your customized care plan.
  • Communicate any questions or concerns you may have after your visit to the MIND program office or to individual practitioners.
  • Consider medical decisions for the future, including advance directives, e.g., Living Will, Durable Power of Attorney, to express your future health care wishes.
